What to Bring to Your Appointment
We aim to make your visits and related arrangements as convenient for you as possible.
These are required when you come in for your appointment:
- Medical insurance card
- Government identification (example: Driver’s License)
To make the process even easier, we suggest that you bring these additional items:
- List of your prescriptions/medications
- List of specialists or other health care providers you see
- List of any recent testing you have had,
- List of questions and concerns, and
- A notebook and pen.
