These are required when you come in for your appointment:

• Medical insurance card
• Government identification (example: Driver’s License)

To make the process even easier, we suggest that you bring these additional items:

• List of your prescriptions/medications,
• List of specialists or other health care providers you see,
• List of any recent testing you have had,
• List of questions and concerns, and
• A notebook and pen.
